Yizkor Memorial Service booklets are used during Yizkor services throughout the year. A new volume is published for each Yom Kippur.  Please note that this is NOT the Jacksonville Jewish Center Yearbook and Member Directory.

• Please use this form to repeat last year's Page Dedication (with or without modification).  
• Please help us make sure we have the most accurate information for the Yizkor book by filling this form out completely.
