Yizkor Memorial Service booklets are used during Yizkor services throughout the year. A new volume is published for each Yom Kippur.  Please note that this is NOT the Jacksonville Jewish Center Yearbook and Member Directory.

PLEASE READ CAREFULLY:

• In order to be included in this year's Yizkor booklet, please submit payment  NO LATER THAN AUGUST 1st.
• Please use this form for requests to repeat last year's entry (with or without modification) or to create a new an entry.  
• To repeat a Page Dedication from last year, please check your email.  You were emailed a separate form directly.   
• If you would like to be on a waiting list to upgrade to a Page Dedication, please indicate that in the form below.
• Please help us make sure we have the most accurate information for the Yizkor book by filling this form out completely.

• If this is a new entry, please list all the names you would like memorialized and the names of those honoring their departed loved ones.
• If this is a repeat entry let us know what changes are needed, if any.
• Note:  The amount due is based on the number of Remembrance Names honored in the Yizkor book.  The first name memorialized is $18 and each additional name is $9 per name.
