2021-2022 Bernard & Alice Selevan Religious School Registration

Membership is required for all returning Religious School families. If you would like to confirm your membership status or become a member of the Center, please contact Tracy Hilbert at thilbert@jaxjewishcenter.org.
Section A: Family Contact Information

If there is a secondary contact, all emails and mailings will go to both contacts. In case of urgent need, the primary contact will be notified first.

Section B: Student Enrollment Information
Sometimes there are obstacles to Jewish education but finances should never be one! We ask that every family commits to some financial contribution, however we are very happy to discuss financial aid with any family who needs it. If you select yes, you will be asked to complete a form which will be reviewed confidentially by the Financial Aid Committee. 
Student 1 Information
This is the same as your child's grade for the 2021-2022 secular school year unless you have spoken with the religious school office and made other arrangements.
As a guide, incoming High School Seniors will be 2022 and incoming Kindergartners will be 2034.
Information will not be shared publicly. Parents/Guardians will be copied on all email communication to students.

Section C: Release Forms
Medical Release
By signing my name below, I grant permission for my child(ren) to participate in all activities that are part of the regular program at the Bernard & Alice Selevan Religious School at the Jacksonville Jewish Center in Jacksonville, FL. I hereby authorize Rabbi Rosenblum, or any person designated by Rabbi Rosenblum, to obtain emergency medical care for my child(ren) in the event such care is necessary. I give permission for my child(ren) to receive emergency medical care by any nurse, doctor, paramedic, or licensed member of a medical staff in a hospital or clinic. I understand that every effort will be made to notify a parent/guardian prior to treatment.

Transportation Release
I acknowledge that field trips may be arranged by the Jacksonville Jewish Center and transportation may include a chartered van/bus or another vehicle driven by a parent/guardian from our Center community.

Media Release
Your child(ren)'s photo will be taken in religious school or at special events. We use these photos on the synagogue website, bulletin boards, in congregational/youth emails, in private Facebook groups, and other publicity materials including, but not limited to, CenterPieces and the Jewish Federation and Foundation of Northeast Florida's Jewish Life Magazine.
Attendance Agreement
I understand that my child(ren) must maintain a minimum of 80% attendance of all scheduled classes while enrolled in grades 2-7.

Payment Agreement
I understand that I am registering my child(ren) for one year and I assume responsibility for the payment of all charges and fees for the entire school year regardless of whether the student is suspended, dismissed or voluntarily withdraws. Should the student be separated from the school for one of these reasons, I understand and agree that no part of the fees paid to the school shall be refunded once the academic year begins.

Section D: Family Engagement
We strive to build powerful Jewish experiences for our students with the support of volunteers for ongoing projects as well as specific programs and events. Beyond the logistical assistance, it is meaningful for our students to see adults modeling Jewish communal engagement.

Section E: Payment Information

In Jewish tradition, it is the responsibility not only of parents, but of the community, to provide an education for our children. The Jacksonville Jewish Center community is committed to this obligation. As such, our religious school tuition only covers a portion of the actual cost of our program. Both fundraising efforts and congregational dues heavily subsidize our children’s education. Obtaining enrollment information early enables us to plan appropriately for the number of students in each grade. In order to ensure that we have accurate numbers, we request that families register on or before July 15th.

PTA Dues: PTA dues of $36 per family will automatically be added to your bill prior to the beginning of school.

B’nei Mitzvah Fees: B'nei Mitzvah fees are assessed separately prior to beginning tutoring in 6th grade and will need to be paid in full before the B'nei Mitzvah.

Section F: Donation Information

The Bernard & Alice Selevan Religious School will never turn away a child due to financial need. While we do charge tuition, much of our operational funds are provided through the generosity of our community. Please consider making an additional contribution below. All donated funds will go towards to enhancement of the Bernard & Alice Selevan Religious School. All donations, of any amount, are greatly appreciated!
