Memorial Plaque

Yahrzeit, the annual commemoration of the anniversary of a loved one’s death, is a traditional act of remembrance

You may want to honor the memory of a loved one by dedicating a memorial plaque to the Jacksonville Jewish Center’s Hannah Setzer Memorial Hall, thus memorializing the name of a loved one for all time.

Memorial plaques are read aloud and dedicated on Yom Kippur at Yizkor services every year. Also, in future years, these names will be included in the perpetual memorials list in the Yizkor Memorial Service Booklet.

To have your loved one’s plaque dedicated on Yom Kippur this coming year, please submit this form NO LATER THAN AUGUST 19th.

Each plaque will include the English and Hebrew names and the date of death according to both the Gregorian and Hebrew calendars.
